He leído y entiendo la actividad que la Iglesia de Dios Pentecostal, M.I. realizará los dias 31 de julio al 2 de agosto del 2025 en la ciudad de Santa Ana; y doy mi consentimiento a que mi hijo/hija participe en esta actividad de la iglesia. Yo me hago responsable de lo que le llegara a pasar a mi hijo/hija (cualquiera que sea la causa) durante este evento y absuelvo de toda culpabilidad o responsabilidad a la Iglesia de Dios Pentecostal, M.I. de lo que le acontezca a mi hijo/hija durante el evento (lesión física, daños a la propiedad, muerte, etc.).
I have read and understand the foregoing release and waiver; and that I’m in consideration of allowing my son/daughter to participate in Pentecostal Church of God’s event: Vacation Bible School, in Santa Ana on July 31st to August 2nd of 2025. I release and waive any claim or legal cause of action that I might have arising out of any personal injury or death of the participant against Pentecostal Church of God, its officials, employees and agents. I further agree to defend, indemnify, and hold harmless the Pentecostal Church of God for and against any claims that the participant may assert or for any personal injury or property damage caused by the participant. I hereby grant my full consent and authorization for the participant to engage in the activity described above.
